Job Description

We are seeking a dedicated Primary Care Physician (PCP) to provide comprehensive care for older adult patients, managing acute and chronic conditions.

• Accountable for patient outcomes, clinical care, and cost-effectiveness within a panel of up to 450 patients.
• Act as a health coach by setting and following up on health goals, simplifying actions for better patient engagement.
• Lead care teams, collaborate with specialists, and coordinate care for hospitalized, SNF, LTC, or homebound patients.
• Ensure compliance with clinical guidelines, quality standards, and departmental objectives.
• Demonstrate strong leadership, communication, and a service-oriented approach, with availability and accessibility for patients.
• Maintain current medical license, board certification or eligibility, and necessary certifications such as DEA and BLS.

Candidates should possess an MD or DO in relevant specialties, be proficient in evidence-based medicine, and committed to continuous improvement.
