• Lead a multidisciplinary team providing comprehensive, patient-centered care with a focus on prevention and chronic disease management.
• Maintain smaller patient panels for deeper relationships, supported by in-house care teams including pharmacists, behavioral health specialists, and care coordinators.
• Provide direct clinical care aligned with best practices, participate in team huddles, manage referrals and care transitions, and ensure accurate documentation with coding support.
• Engage in quality improvement, policy discussions, and enjoy minimal call duties.

• Qualifications: MD or DO, active license, board eligibility in Internal Medicine, Family Medicine, or Geriatrics preferred, with 2+ years' experience and Medicare Advantage knowledge advantageous.
