Details:
Meet with patients for one-on-one support and guidance.
Assist patients in developing recovery plans, coping methods, and rehabilitation strategies in collaboration with the behavioral health team.
Monitor patients' behavior, schedules, medications, and activities, providing feedback to the behavioral health team.
Liaise regularly with family members and medical professionals.
Generate recovery reviews and performance documents to track the rehabilitation process.
Assist in arranging medical appointments, therapy sessions, support groups, and other resources based on patient needs.
Requirements:
High School Diploma or GED required.
Completed Peer Support Training with a valid certificate.
Experience in community engagement, with an emphasis on building and maintaining partnerships.
Valid Fingerprint Clearance Card.
Any combination of experience and education that provides the required skills may be considered.
