Responsibilities:
- Patient assessments and monitoring.

- Treatment planning and implementation.
- Collaboration with interdisciplinary team (physicians, therapists, etc.).
- Case management and care coordination.
- Providing skilled nursing care in various settings (hospital, home health, hospice).
- Maintaining accurate patient records and documentation.
Required Skills & Certifications:
- Clinical assessment, medication administration, and treatment planning skills.
- Active Registered Nurse license in the state of employment.
- Bachelor's degree in Nursing or related field.
- Minimum two years of experience in hospital, home health, or hospice care.
- Excellent communication and interpersonal skills.
- Ability to work effectively in a fast-paced environment.
- Driver's license, car insurance, and personal automobile for field work (if applicable).
Preferred Skills & Certifications:
- Experience in Medical/Surgical, ICU, ER, Psychiatric, Labor & Delivery, PACU, or Operating Room settings.
Special Considerations:
- On-call responsibilities are expected 1-2 days per week and rotating weekends.
- First-time RNs are accepted.
Scheduling:
- Schedule will include rotating weekends and on-call shifts (1-2 days/week). Specific shift assignments will be determined by the employer.
